New Patients

We are taking on NEW patients, if you would like to join our surgery then please either:

  • Collect a hard copy of our registration pack from the GP surgery
  • Download and fill in a GMS1 form AND the New patient Questionnaire form which can be found below.

Both forms are essential for your registration to be processed. If you do NOT complete both parts your request will not be accepted. Once completed you will need to bring them to the GP surgery to be processed.

Although it is not essential we do ask that when registering you provide us with proof of identity and address as this helps the admin team when submitting your registration to the local authorities. Please be reassured that we do not retain these forms of ID. They are securely destroyed.

We do ask that if you are completing your registrations in person that each individual registering attends when you bring your completed forms.


We ask that if there are any children registering you provide us with a copy of their immunisation history (the red book is preferable).

If any persons registering take any regular medications we advise that you obtain a month’s supply from your previous GP before submitting your registration with us as we do not want anybody going without any medication during your transition to our practice.

You will be offered a new patient check when registering with our practice this is to introduce you to the practice, obtain a family history and to continue any ongoing care if necessary.

The practice can only refuse your application if it has fair and reasonable grounds to do so or you live outside the practice area.

When you register, it’s helpful to have your NHS number.

You can use the NHS website to find your NHS number.

However, you do not need proof of address or immigration status, ID or an NHS number to register at the practice.
